South Carolina health inspectors found multiple violations at restaurants in the Midlands in May, from roaches found in a meat market to kitchens with flies and a bag of dripping raw chicken juices. The state each month releases grades and inspection reports for food establishments across Midlands counties.
Here are the restaurants that received some of the lowest ratings in May.
Hardee’s at 1733 Whiskey Road in Aiken
Hardee’s had an inspection on May 7 and got a C grade.
An inspector observed flies throughout the kitchen. Also observed was a dead fly on the serving counter and several dead flies throughout the dining room.
Excessive grease accumulation was seen on the floors throughout the facility. Grease and trash debris was seen in the back storeroom. Both bathroom toilets were not being properly maintained. An employee was seen beginning shift without washing hands.
The restaurant had a required followup inspection on May 29 and got an A grade.
Blaney’s Backyard Grille at 1244 Pine St. in Kershaw
Blaney’s Backyard Grille had an inspection on May 1 and got a C grade.
An inspector observed an employee handling dirty dishes and then touch clean dishes without proper handwashing first. Also observed were dirty dishes stored as clean.
Several buckets of made sauces were seen kept after their expiration dates. Buckets of food were seen stored on the floor of the walk-in cooler. A sticky residue was seen on the ice machine in the bar area.
The restaurant had a required followup inspection on May 9 and got an A grade.
Boho Pizza and Brew at 2415 Augusta Hwy in Lexington
Boho Pizza and Brew had an inspection on May 30 and got a C grade.
An inspector observed numerous flies and rat droppings throughout the facility. Also observed were several fly strips over clean dishes.
Meat was seen sitting out to thaw in standing water. Pizza sauce and cooked chicken wings were seen stored without date marks. Certain foods were seen not kept at proper holding temperatures. Employees were seen with dirty aprons.
A required followup inspection has not yet been held.
Flaming Grill Supreme Buffet at 115 Afton Ct in Lexington
Flaming Grill had an inspection on May 7 and got a C grade.
An inspector observed multiple dead inspects throughout the facility. Also observed was raw chicken stored directly on the floor.
Certain foods were seen not kept at proper holding temperatures. Large pans with a crusty buildup on them were seen stored as clean. Raw chicken was seen stored above raw beef in the walk-in cooler.
The restaurant had a required followup inspection on May 22 and got an A grade.
San Jose Mexican Restaurant at 498 Piney Grove Road in Lexington
San Jose Mexican Restaurant had an inspection on May 6 and got a C grade.
The restaurant had to temporarily close because its main walk-in cooler was in such poor condition and not operating properly.
An inspector observed an employee handle dirty dishes without washing hands. Also observed an employee handle raw meat and then touch handles of equipment without washing hands.
Dented cans were seen stored with in-use inventory. Food debris was seen on the exterior surfaces of pans stacked together and on knives stored as clean. Certain foods were seen not kept at proper holding temperatures.
The restaurant had a required followup inspection on May 16 and got an grade.
San Jose at 121 Dreher Road in Lexington
San Jose had an inspection on May 3 and got a C grade.
An inspector observed an employee cutting lettuce with bare hands.
Meats were seen stored improperly in the freezer. Certain foods were seen not kept at proper holding temperatures. Grease pipe buildup was seen coming from hoods and flowing to an overfilled bucket, resulting in grease spilling onto the ground outside.
The restaurant had a required followup inspection on May 16 and got an A grade.
2 Gingers at 245 Bush River Road in Richland
2 Gingers had an inspection on May 1 and got a C grade.
An inspector observed an employee handle cooked bread with bare hands.
Foreign particles were seen in containers of salt and MSG. Carbon buildup was seen on multiple pots stacked with clean pots. Food debris was seen on dishes stored as clean. Certain foods were seen not kept at proper holding temperatures.
Heavy dust and debris was seen on the ceiling throughout the kitchen.
The restaurant had a required followup inspection on May 13 and got an A grade.
Angelo’s Zesto at 4001 N Main St in Richland
Angelo’s Zesto had an inspection on May 15 and got a C grade.
An inspector observed flies and drain flies throughout the facility.
Raw chicken was seen stored exposed in the walk-in cooler. The cashier was seen handling money before then scooping ice for a sold beverage. Dishes and pans soiled with foil and debris were seen stored as clean.
Certain foods were seen not kept at proper holding temperatures. Food, grime and debris was seen on floors and walls throughout the facility.

Asian Express at 1935 Broad River Road in Richland
Asian Express had an inspection on May 13 and got a C grade.
An inspector observed an employee handle raw product in a container that had been soiled without washing hands and without changing gloves.
Eggs and raw meat were seen stored above produce. A cutting board was seen stored as clean without being sanitized.
Certain foods were seen not kept at proper holding temperatures. Multiple containers of prepared foods and sauces were seen without a date marking system.

Legacy Bar & Grill at 215 Oneil CT in Richland
Legacy Bar & Grill had an inspection on May 14 and got a C grade.
An inspector observed an employee change from handling raw chicken to handling containers for ready to eat food and to grabbing handles of equipment without washing hands. Also observed were onions stored in a box previously used for raw chicken.
Dishes with food debris were seen stored as clean. A dark buildup was seen in the ice machine. Certain foods were seen not kept at proper holding temperatures. Deep grooves and dark buildup were seen on cutting boards.

Logan’s Upper Crust at 843-4 Polo Road in Richland
Logan’s Upper Crust had an inspection on May 29 and got a C grade.

M Vista at Lady St in Richland
M Vista had an inspection on May 17 and got a C grade.
An inspector observed an employee at the prep line cooler using the same spoon for multiple raw products. Also observed was raw chicken on the back prep kitchen table next to whole carrots.

The restaurant had a required followup inspection on May 30 and got an A grade.
Rivera’s Food at 1807 Decker Blvd in Richland
Rivera’s Food had an inspection on May 15 and got a C grade.
An inspector observed liquid dripping from large bags of raw chicken into a container of bread crumbs. Also observed were dead roaches in the meat market.
Certain foods were seen not kept at proper holding temperatures. A bag of raw chicken was seen stored directly on the floor. Standing water was seen in the bottom of the meat display case. Grooves and dark buildup were seen on cutting boards.
The restaurant had required followup inspections on May 22 and 29 and earned two more C grades. A third followup inspection has not yet been held.
